The muscle preventing the backward flow of stomach contents is sensitive to the amount of acid in the stomach. When more acid is being produced in the stomach, the tighter the muscle contracts, preventing the reverse flow of stomach contents. If there is not enough acid being produced by your stomach, the muscle will be more lax and more likely to allow reverse flow. If you experience heartburn, on an empty stomach, your body is producing too much gastric acid and this could be a result of interference with the parasympathetic system. Both are causes of too little or too much gastric acid production.
